Eligible Applicants

We offer braces at a lower cost to low-income families. Youth applying for our program must, at minimum, meet the following guidelines and be motivated to want this type of treatment.

To be eligible for our program, you must:

•  Submit a complete application and $25 application fee

• Be age 11-18 and have good oral hygiene (including regular dental visits and cleanings).

• Be in need of orthodontic treatment that can be delivered as a one-stage process.

• Be willing to pay $500 toward orthodontic treatment.*

• Complete an application and attach all requested documents, including a completed dental referral form and a copy of the most recent tax return documentation.

• Have an annual household taxable income (line 43 on 1040 form) at or below 200% of the Federal Poverty Guideline. To verify that you meet the SCL income guidelines, please visit www.smileschangelives.org/qualify.

• Be willing to follow all program rules (attend scheduled appointments and follow orthodontist’s instructions for care of teeth and braces).

Applicants who are already wearing braces are not eligible for the program.

If accepted into the program, SCL provides for orthodontic treatment ONLY. Extractions, cleanings, oral surgery or other treatment that may be necessary before, during or after orthodontic treatment are the financial responsibility of the patient’s parents or legal guardians.
