Apply For Braces

Smiles Change Lives and participating orthodontists are dedicated to providing braces for youth from low-income families. We believe self-esteem and confidence is very important to the development of a child. SCL orthodontists provide quality treatment and healthy smiles to the most qualified and deserving kids.

Please apply to the program even if you don't see a provider in your area. SCL is actively recruiting orthodontists to serve youth across the U.S.

Our general guidelines include:

• Send a complete application with all required documents and the $25 application fee
• Youth applying to the program must be between the ages of 11-18 years old
• Not already wearing braces
• Have a household taxable income at or below 200% of the Federal Poverty Guidelines (line 43 on the 1040 form; visit www.smileschangelives.org/qualify for more infomation)
  
• Family must be willing to pay $500 toward program participation*
• Youth applying must be motivated toward receiving orthodontic treatment

*A limited number of partially subsidized spots are available in some areas.

3. Mail to our main office, with all required documents, at the address listed on the application

2. Fill out completely and return (along with all required documents) to the address listed on the application.
