Smiles Change Lives - Applicants

Do you qualify?

1040 Form

1.  Based on your taxable income (line 43), find where you meet the SCL income guidelines. If you are receiving Social Security as a form of income, please click here to see an example of an awards letter you will need to submit in place of your taxes.
NOTE: Your taxable
income must be at or below 200% of Federal Poverty Level to qualify financially.

FPL

2. If your taxable income (line 43) falls at or below 100% of the Federal Poverty Level, you may qualify for one of a limited number of partially-subsidized placements that may be available in some areas. These placements are in high demand and are based on the donations received and the number of qualified applicants in any area. SCL cannot guarantee a subsidized placement.

If a subsidized placement is available at the time your child is approved for our program and your income falls at or below 100% of the Federal Poverty Level, you will be notified and your payment will be reduced.

However, if your child is approved for our program and there are no subsidized placements available at that time, you may choose to pay the full $500 program fee, or you may choose to withdraw from our program.

Partial subsidies are available because of donors in your area who choose to take part in our Adopt-A-Smile® program.
